REQUIREMENTS
Application Form in A4 size paper (Computerized, handwritten not allowed, please fill up from PDF file to avoid any disagreements.)
1 pc. passport size colored picture (3.5x4.5cm) white background & taken within 6 months
Passport (valid within 6 months)
Photocopy of Passport Bio-page in A4 size paper
Original & Photocopy of valid visa and arrival stamps to OECD member countries for the past 5 years in A4 size paper
ADDITIONAL REQUIREMENTS
IF YOU ARE AN EMPLOYEE:
Original Certificate of Employment
 must be issued within 3 months and include applicant's position, date hired, compensation, company name & address, HR landline number (cell phone number is not allowed), HR e-mail address.
IF YOU ARE A BUSINESSMAN:
Photocopy of Latest Business Registration from SEC or DTI in A4 size paper

Tourist Visa and Service Fee (Min. of 3 pax) | PHP 2,400.00 PER PERSON |
Tourist Visa and Service Fee (for 1 pax) | PHP 3,500.00 PER PERSON |
PROCESSING TIME
Regular visa processing will take 7 to 14 working days after filing in the KVAC. However, in case a high volume of applications is received, the processing period may be extended accordingly
IMPORTANT REMINDERS:
Bring ORIGINALS and PHOTOCOPIES of ALL required documents
Bring both your original old and most recently issued passport
Photocopies of the first page and last page of both the old passport and most recently issued passport are required. Photocopies of previous visas and immigration stamps from countries previously visited are required
Each applicant must have their own file. Please ensure that for Families with shared documents, you must provide individual copies of documents for each member
Fill-up your application form accurately. Indicate N/A for items that are not applicable. Do not forget to affix your signature on the last box of the application form
